This is more a question for comp.protocols.dicom

Acculite, ImageJ, DICOM Works, MRIcro and IDL probably look at the
NumberOfFrames tag (0028,0010).

Your PACS workstation could be trying to work out the modality, image
type and arrangement of frames and is getting confused at a certian
point or will not accept that type of image. It is also possible that
your file is simply not DICOM compliant. You need to be careful when
you alter the dimensions of a DICOM image. Such a change may require
the alteration of many different DICOM tags. You need to be sure that
your SOP class supports 4D.

I'd recommend doing some reading on DICOM and/or contacting your PACS
vendor for assistance. Perhaps some example 4D images might help too.
