I got the following advice from ittvis, which fixed the problems noted
above.

Hello Mark,

You can probably get your license manager service running again by
deleting what is in the StartupItems now and rerunning the
lmgrd_install script. You can do this from a terminal window:

sudo ./lmgrd_install

When this command is done you should be able to see its result by
typing the following command:
ls /Library/StartupItems/IDL_LMGRD

Does this show that there are two items in that 'IDL_LMGRD' directory:
'IDL_LMGRD' and 'StartupParameters.plist'? If you were to look at that
'IDL_LMGRD' file (with the command:
cat /Library/StartupItems/IDL_LMGRD/IDL_LMGRD
for example), you would see that it is simply a copy with a new name
of the 'sys5_idl_lmgrd' file that you just edited. Then either reboot
or manually launch the license manager in an x11 terminal.

However, I suspect your issues with Shadesurf are actually an
incompatibility between the update and the older version of IDL. We
know that the 2009-001 update security update for intel and ppc mac
broke parts of IDL 6.4 and older versions. You might find the TV and
TVSCL functions also do not work. The resolution is to update to a
supported version of IDL or to roll back the security update. You may
be able to just reinstall X11.1.1.3:

Let me know if this helps.

Best Regards,
