By the way, I've found that the link (everything between the
two backward quotes) has to be on the same line in the file.
This sometimes makes line breaks in the actual file itself
awkward, but the links are not recognized otherwise.

Like this:

`Link text <http://foo.bar>`

with the little "backward quote", or whatever you call
that character.

Yes, there is also a feature request to allow this to span multiple lines:
