Try this:

SZ = SIZE(IMAGE)
SYM_IMAGE = FLTARR(2*SZ(1)-1,2*SZ(2)-1)
SYM_IMAGE(SZ(1)-1,SZ(2)-1) = IMAGE
SYM_IMAGE(0,SZ(2)-1) = REVERSE(IMAGE,1)
SYM_IMAGE(SZ(1)-1,0) = REVERSE(IMAGE,2)
SYM_IMAGE(0,0) = REVERSE(REVERSE(IMAGE,1),2)
